Continuing Education Information

All of the forms needed to get your district CE's approved are listed here as separate files. All of them are required for approval.  You can print them off as you need them. 

Use the following codes for all CE Approvals:
RN - 50-777
CDMS, CCM, CRC - 87075

• Application (Microsoft Word Format)
• CE Data Sheet (Microsoft Word Format)
• Roster (Microsoft Word Format)
• Certificate of Attendance (Microsoft Word Format)
• General Evaluation Form Summary (Microsoft Word Format)
• Course Evaluation (Microsoft Word Format)
  
  Please note the following requirements:
• Only the CE Chairperson may approve CE Offerings
• All CE Applications must be sent in (via email, snail mail, or fax) at least 30 days prior to the offering for approval
• There are no automatic approvals with use of these forms or approved codes for offerings
• Sign in sheet (roster) and evaluations must be sent in within 15 days post-offering to complete the CE Approval process
• Unfortunately, failure to comply with the above will result in non-approval of your CE Offerings
• No exceptions!
